This is a mechanism module, not a specific disease. Do NOT create a "GLP-1 Receptor Agonism" Disease entry. Disorder entries reference individual nodes via conforms_to, for example "glp1_receptor_agonism#PKA and Epac2 Effector Engagement".
Scope. The module opens at the drug-receptor interaction and stops at the organism-level outcomes. The disease state the therapy acts on - beta-cell failure, insulin resistance, hypothalamic dysregulation, positive energy balance - belongs to the conforming entry, as does the end-organ damage of chronic hyperglycaemia, which `diabetic_vascular_complications` already carries.
What this module refuses to claim. It names no dominant mechanism for weight loss. PMID:42083883 assesses eight candidate mechanisms and concludes that the evidence supports no single dominant one, and it finds the liraglutide appetite effect moderate, not dose-dependent, and transient. A conforming obesity entry should therefore conform at the receptor or cAMP step rather than asserting that its weight loss runs through a named arm. Writing one in would be exactly the over-claim the evidence rules out.
The emesis arm's attachment point is a deliberate judgement, not an oversight. Area postrema GLP-1R activation hangs off the trigger rather than the central effector because the direct evidence there (murine, PMID:40622911) demonstrates postsynaptic AMPA-receptor dependent coupling and does not place the step below cAMP/PKA. If a cAMP-dependent account of that site is established, the edge should move.
Ontology bindings. Nodes bind GO and CL terms only; no CHEBI term is placed on cAMP itself, following the convention recorded in `cardiac_camp_pka_calcium_handling`. Epac2 is named in prose rather than bound, because the node's molecular function is the cAMP-dependent kinase activity GO:0004691 and no single GO term covers both effectors. GIPR (hgnc:4271) is deliberately not bound on any node: co-agonists reach this module through their GLP1R half only.
GLP-1 Receptor Agonist Binding and Activation
trigger
An incretin mimetic occupies and activates GLP1R, a class B G protein-coupled receptor. Unlike the receptor-blockade modules, the drug here reproduces an endogenous signal rather than removing one, and it does so at a concentration and duration the native hormone never reaches: GLP-1 itself is degraded within minutes, while the agonists are engineered to resist that degradation. The receptor is expressed well beyond the islet - hindbrain, hypothalamus, stomach, vagal afferents - which is why one receptor action produces the several arms below.

Gs Coupling and Adenylyl Cyclase Activation
The activated receptor engages Gs and stimulates adenylyl cyclase.

Area Postrema GLP-1R Neuronal Activation
The same agonist engages GLP1R on area postrema neurons. Attached here rather than below the cAMP step because the coupling demonstrated at that site is postsynaptic and AMPA-receptor dependent.

"Although GLP-1 is established to be a cAMP-elevating agent, these studies demonstrate that protein kinase A (PKA) is not the only cAMP-binding protein by which GLP-1 acts."
Establishes the receptor's second-messenger consequence that the next node carries, and flags that the effector step is not PKA alone - which is why this module's central effector names Epac2 beside it.
"GLP-1 RAs are injectable peptides that resemble the structure and function of endogenous incretin GLP-1, but as they are not deactivated by the dipeptidyl peptidase-4 (DPP-4), their half-life is prolonged compared with native GLP-1."
Sources this node's distinctive claim - that the drug reproduces the endogenous signal at an exposure the native hormone cannot reach, because it escapes the protease that clears GLP-1.
Gs Coupling and Adenylyl Cyclase Activation
amplifier
The agonist-bound receptor catalyses nucleotide exchange on Gs-alpha, which stimulates adenylyl cyclase and raises intracellular cAMP in the cell that carries the receptor. No CHEBI binding is placed on cAMP itself, per the module convention that nodes bind GO and CL terms only.

PKA and Epac2 Effector Engagement
Raised cAMP is read by both of its effector proteins.
Show evidence (1 reference)
"Although GLP-1 is established to be a cAMP-elevating agent, these studies demonstrate that protein kinase A (PKA) is not the only cAMP-binding protein by which GLP-1 acts."
States the cAMP rise this node asserts, in the cell type the islet arm depends on.
PKA and Epac2 Effector Engagement
central effector
Raised cAMP engages two effectors, not one: protein kinase A, and the cAMP-regulated guanine nucleotide exchange factor Epac2, which couples the cAMP rise to calcium-dependent exocytosis and to calcium release from intracellular stores. This is the module's key conformance target for the metabolic arms - the step the islet, gastric and feeding arms share - and it is the mirror image of `beta_adrenergic_receptor_blockade`, whose central effector is the same cascade running down rather than up. The emesis arm is deliberately not placed below it.

Glucose-Dependent Potentiation of Insulin Secretion
In the beta cell the cAMP effectors amplify secretion already initiated by glucose.
-
Suppressed Glucagon Secretion
Alpha-cell glucagon output falls.
-
Slowed Gastric Emptying
Gastric motor activity is inhibited, delaying nutrient delivery to the small intestine.

Reduced Food Intake
Hypothalamic and hindbrain GLP-1R circuits reduce intake.

"Two variants of Epac (Epac1 and Epac2) are expressed in beta-cells, and downregulation of Epac function diminishes stimulatory effects of GLP-1 on beta-cell Ca(2+) signaling and insulin secretion."
Supports naming Epac beside PKA: removing Epac function blunts the insulin-secretory effect, so the effector step is not PKA alone.
Glucose-Dependent Potentiation of Insulin Secretion
effector
The cAMP effectors amplify insulin secretion that glucose has already initiated, by reloading the immediately releasable granule pool and sensitising exocytosis to calcium. The glucose-dependence is the clinically decisive property and the reason this class does not cause hypoglycaemia the way a sulfonylurea or exogenous insulin does: below a glucose threshold the amplification does not occur. A conforming entry that models beta-cell failure substitutes its own account of the secretory defect this potentiation acts on.

Improved Glycaemic Control
More insulin at the prandial peak lowers postprandial glucose.
Show evidence (2 references)
"Among the most well-known and studied effects of GLP-1 is its impact on the pancreatic islet, where the activation of GLP-1-associated signaling pathways results in increased insulin secretion and reduced glucagon production."
States both islet arms of this module in one sentence.
"The first mechanism involves the enhancement of the reloading of the insulin pool available for immediate release, occurring only in the presence of incretin and at a certain glucose threshold."
Sources the granule-pool mechanism and, with it, the glucose threshold that makes this node's potentiation glucose-dependent rather than unconditional.
Suppressed Glucagon Secretion
effector
Alpha-cell glucagon output falls, reducing the hepatic glucose production that glucagon drives. Whether the effect is direct on the alpha cell or mediated through beta- and delta-cell paracrine signalling is not settled here, and the module does not assert which; the node records the output change the cited review states.

Improved Glycaemic Control
Less glucagon means less hepatic glucose output, chiefly between meals.

"Among the most well-known and studied effects of GLP-1 is its impact on the pancreatic islet, where the activation of GLP-1-associated signaling pathways results in increased insulin secretion and reduced glucagon production."
States the reduced glucagon output this node records.
Slowed Gastric Emptying
effector
Gastric motor activity is inhibited and nutrient delivery to the small intestine is delayed, blunting the postprandial glucose excursion and contributing to satiety. The effect attenuates with continued exposure to a long-acting agonist, which is one reason the weight-loss contribution of this arm is contested rather than settled.

Improved Glycaemic Control
A slower nutrient influx lowers the postprandial glucose peak.

Weight Loss
Delayed emptying contributes to reduced intake, by a share this module does not quantify.
Show evidence (1 reference)
"These agonists/analogues of GLP-1 slow down gastric emptying, modulate calorie intake by increasing the sense of satiety"
States this node and the satiety arm beside it, for the agonist class rather than for native GLP-1.
Reduced Food Intake
effector
GLP-1R circuits in the hypothalamus and hindbrain reduce food intake and subjective appetite. The node is deliberately modest: the cited critical review finds the appetite effect of liraglutide to be only moderate, not dose-dependent, and transient, so this arm is recorded as one contributor among several rather than as the mechanism of weight loss. No cell type or anatomical site is bound: the cited sources name satiety and calorie intake without naming a nucleus or a neuronal population, and the receptor is expressed at several of the sites that could carry this arm, so binding one of them would assert a route the evidence here does not establish.

Weight Loss
Reduced intake contributes to the energy deficit.

"These agonists/analogues of GLP-1 slow down gastric emptying, modulate calorie intake by increasing the sense of satiety"
Sources the intake and satiety effect this node records.
"Overall, these findings suggest that liraglutide produces only a modest, nondose‐dependent appetite effect that appears to be transient."
Bounds the node. A critical review of the mechanisms finds the appetite effect moderate and transient, which is why this arm is not offered as the dominant route to weight loss.
Improved Glycaemic Control
consequence
The organism-level metabolic outcome the diabetes indication rests on: lower postprandial and fasting glucose, reached through three arms at once - more glucose-dependent insulin, less glucagon, and slower nutrient delivery. Because the insulin arm is glucose-dependent, the improvement carries a low intrinsic hypoglycaemia risk.
Show evidence (2 references)
"Among the most well-known and studied effects of GLP-1 is its impact on the pancreatic islet, where the activation of GLP-1-associated signaling pathways results in increased insulin secretion and reduced glucagon production."
Sources the two islet contributions this outcome aggregates. Graded INDIRECT because the quoted sentence asserts the upstream islet mechanism, not the glycaemic outcome this node records; the step from one to the other is short but it is a step.
"Although all of these agents potently reduce haemoglobin A1C (HbA1c), there are unique features and fundamental differences among them"
Sources the glycaemic outcome itself, across the class rather than for one agent.
Weight Loss
consequence
Sustained weight loss, the outcome the obesity indication rests on. The module deliberately does NOT name a dominant upstream arm: a critical review of eight purported mechanisms - appetite, chemosensory function, cravings, food noise, gastric emptying, the microbiome, incretin activity and energy expenditure - concludes that the evidence supports no single dominant mechanism and that a combination of subtle effects is likely. A conforming obesity entry should therefore conform at the receptor or cAMP step and not assert that its weight loss runs through one named arm.
Show evidence (2 references)
"GLP-1RA medications have led to unprecedented, nonsurgical levels of weight loss."
Sources the outcome this node records - that sustained weight loss occurs on these drugs.
"Current evidence does not support a single dominant mechanism; a combination of subtle effects may underlie the efficacy of these medications."
Graded NO_EVIDENCE for this node's claim. The sentence bears on which upstream arm drives the outcome, not on whether the outcome occurs, and it is quoted here as the reason the module names no dominant arm rather than as support for weight loss.
Area Postrema GLP-1R Neuronal Activation
adverse effect
GLP1R-expressing neurons of the area postrema, a circumventricular structure outside the blood-brain barrier, are activated by the circulating agonist. In mice this is the step that produces emesis: ablating GLP-1R in the area postrema abolishes the response, while cutting the vagal afferents does not, and the coupling is postsynaptic and AMPA-receptor dependent. That last point is why this node hangs off receptor activation rather than off the module's cAMP effector - the evidence at this site does not place it below that step.

Nausea and Vomiting
Activation of this emetic centre produces the class's dose-limiting symptom.
Show evidence (1 reference)
"genetic ablation of GLP-1 receptor (GLP-1R) expression in the AP completely abolished this response, establishing AP GLP-1R as the critical mediator of GLP-1-associated emesis"
Direct genetic evidence that this site's receptor is necessary for the adverse arm. Mouse, so the translational step to human nausea is an inference the consequence node carries.
Nausea and Vomiting
consequence
The dose-limiting adverse effect of the class, and the usual reason for dose reduction or discontinuation. It is a mechanism rather than an idiosyncrasy: the same receptor that delivers the metabolic benefit mediates the symptom, which is why tolerability is managed by slow up-titration rather than by avoiding the target. The human attribution rests on the clinical record; the circuit evidence cited upstream is murine.
Show evidence (1 reference)
"Although glucagon-like peptide-1 (GLP-1) analogs have been clinically approved for type 2 diabetes mellitus (T2DM) and obesity treatment for an extended period, their associated adverse effects of nausea and vomiting remain unsolved."
States that nausea and vomiting are an established, unresolved adverse effect of the approved drugs in people, which is the claim this node makes.
