Hypotrichosis 11

Mendelian MONDO:0014027 Pathograph 9 Show in embeddings browser Hypotrichosis Simplex Spliceosomopathy

Hypotrichosis 11 (HYPT11) is an autosomal dominant form of hypotrichosis simplex, a nonsyndromic isolated hair loss, caused by heterozygous variants in SNRPE. SNRPE encodes SmE, one of the seven Sm proteins that form the ring-shaped core of the spliceosomal U snRNPs. In the founding report, affected members of a Spanish family, and simplex cases from the UK and Tunisia, had scanty or absent eyebrows and eyelashes and a degree of scalp and body hair loss present from birth that varied widely within the family, from slight thinning to complete loss of scalp and body hair, with skin, nails, teeth and sweating normal. The start-codon variant c.1A>G (p.Met1?) segregated in the family and arose de novo in the UK case; the Tunisian case carried the Sm-motif missense variant c.133G>A (p.Gly45Ser). In transfected cells both variants reduced the soluble, snRNP-incorporable fraction of SmE, but splicing of minigene reporters and in patient lymphocytes appeared normal. How a defect in a ubiquitous core splicing protein produces a hair-restricted phenotype is unknown.

Discussions and Knowledge Gaps

1
Why does a heterozygous variant in SmE, a core protein of every spliceosomal U snRNP and of the U7 snRNP, affect only hair, and does it change splicing in the hair follicle?
KNOWLEDGE GAP OPEN snrpe_hair_restriction
SmE is expressed in all scalp cell types, yet the disease spares skin, nails, teeth and sweating. Minigene reporters and patient lymphocytes showed no major splicing abnormality, so the mis-splicing step that the spliceosomopathy module places downstream of the variant has not been observed for this disease, and the authors also raised a U7 snRNP (histone mRNA processing) contribution. A dominant Snrpe mouse mutant has no obvious hair phenotype, so no animal model exists. Other SNRPE variants cause microcephaly, which shows that different alleles reach different tissues. Transcriptome profiling of patient hair follicle cells, which the founding report proposed, has not been reported.

Heterozygous SNRPE Variant
A heterozygous germline SNRPE variant alters SmE, a core Sm protein of the spliceosomal U snRNPs. The start-codon variant c.1A>G and the Sm-motif missense variant p.Gly45Ser are the variants with functional data. Evidence is from human genetics (linkage, segregation, de novo occurrence).

"we identified the heterozygous mutation c.1A>G (p.Met1?) in SNRPE that results in loss of the start codon of the transcript."
The trigger is a heterozygous germline variant in this spliceosome component.
Reduced Soluble SmE for U snRNP Incorporation
In transfected HEK293T cells both disease variants lowered the amount of soluble SmE: c.1A>G gave full-length protein from the GTG codon at lower levels plus a mostly insoluble N-terminally truncated form, and p.Gly45Ser was incorporated into the Sm core less efficiently. The authors proposed that the variants act by reducing the soluble fraction available to U snRNPs. In lymphocytes from a c.1A>G carrier, full-length SmE was incorporated into the Sm core with near-normal efficiency, so the reduction has not been shown in patient cells. Evidence is in vitro (HEK293T overexpression, patient lymphoblastoid cells).

PMID:23246290 SUPPORT In vitro / ex vivo
"However, in the case of p.Gly45Ser, the reduction in the incorporation efficiency is more substantial"

PMID:23246290 REFUTE In vitro / ex vivo
"only full-length protein is produced and incorporated into the Sm core with almost equal efficiency to the wild-type"
In patient lymphocytes carrying c.1A>G, Sm core incorporation was near normal, so reduced incorporation is not demonstrated in patient cells.
Impaired Hair Follicle Hair Production
Hair follicles produce too little or no hair, most consistently at the eyebrows and eyelashes, while skin, nails, teeth and sweating are spared. SmE is present in the nuclei of all hair follicle, epidermal and dermal cells of human scalp, so the restriction is not explained by expression. Whether the follicle defect is a subtle splicing change, reduced SmE dosage, or a U7 snRNP (histone mRNA 3' end processing) defect has not been tested in hair follicle cells. Evidence is human clinical phenotype plus human scalp immunohistochemistry.

  prompt: >-
Why does a heterozygous variant in SmE, a core protein of every spliceosomal
U snRNP and of the U7 snRNP, affect only hair, and does it change splicing in
    the hair follicle?
  rationale: >-
SmE is expressed in all scalp cell types, yet the disease spares skin, nails,
teeth and sweating. Minigene reporters and patient lymphocytes showed no major
splicing abnormality, so the mis-splicing step that the spliceosomopathy
module places downstream of the variant has not been observed for this
disease, and the authors also raised a U7 snRNP (histone mRNA processing)
contribution. A dominant Snrpe mouse mutant has no obvious hair phenotype, so
no animal model exists. Other SNRPE variants cause microcephaly, which shows
that different alleles reach different tissues. Transcriptome profiling of
patient hair follicle cells, which the founding report proposed, has not been
    reported.
